The services provided by VitalityHaus, including intravenous (IV) infusion therapy, intramuscular (IM) injections, and related wellness treatments, are intended to support general health and well-being. These services are not a substitute for professional medical diagnosis, treatment, or care from a licensed physician. VitalityHaus does not diagnose, treat, cure, or prevent any medical conditions or diseases.

Clients are encouraged to consult with their primary healthcare provider before beginning any treatment, particularly if they have pre-existing medical conditions or are currently taking prescription medications.

Informed Consent

By receiving services at VitalityHaus, you acknowledge and agree to the following:

  • You provide informed consent to receive IV infusions, IM injections, and other wellness services.

  • You understand the nature of the services, potential risks, and that results may vary.

  • You confirm that you are 18 years of age or older.

  • You agree that you have voluntarily provided complete and accurate medical history, including information about any chronic conditions, medications, supplements, surgeries, and known allergies.

Risks and Complications

Although our procedures are generally safe when administered by trained professionals, all medical treatments involve certain risks. Possible side effects or complications may include, but are not limited to:

  • Pain, swelling, bruising, or infection at the injection site

  • Vein irritation, inflammation, or clotting

  • Allergic reactions, including severe reactions such as anaphylaxis

  • Electrolyte imbalances or fluid overload

  • Nerve damage or tissue injury from catheter misplacement

  • Rare but serious events such as cardiac arrest or death

VitalityHaus takes precautions to minimize these risks, but not all complications can be anticipated or prevented.

Limitation of Liability

By participating in treatments offered by VitalityHaus, you agree to release, indemnify, and hold harmless VitalityHaus, its owners, employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury—including death—that may be sustained as a result of the services provided.

Emergency Situations

VitalityHaus does not provide emergency medical care. In the event of a medical emergency or adverse reaction following a treatment, you agree to seek immediate care from a hospital or emergency service provider. You also agree to notify VitalityHaus of any complications or additional care received.

Payment and Insurance

VitalityHaus is a self-pay facility. All services must be paid in full prior to administration. We do not accept insurance, including Medicare, and we do not submit claims on behalf of clients unless otherwise required by law.

Right to Refuse Services

VitalityHaus reserves the right to refuse or discontinue services at any time, with or without cause, including but not limited to:

  • Falsification of information

  • Signs of intoxication or substance use

  • Medical contraindications

  • Disruptive or inappropriate behavior

Privacy & Medical Records

Your medical information is handled in accordance with applicable privacy regulations. You consent to the release of your medical information as necessary to coordinate treatment or respond to emergencies, including sharing details with other healthcare providers or emergency personnel.
